- Additional Information: Candidates must include RTO (Request for Time Off) at the time of submission and respond to whether they have previously worked at a Providence facility as a permanent employee, on a travel contract, or as registry/per-diem, including locations and dates. An LA fire card obtained within the last 30 days is required. References: two references covering the last 1 year (6 months for CNAs) are required and must be provided during the compliance process.
- Schedule: 8-hour days, 40 hours per week, starting on 07/07/2026.
- Orientation Hours: Not specified.
- Additional Responsibilities: Must be able to independently perform Modified Barium Swallow Studies. Must commit to working 3 of the 5 holidays (Thanksgiving, Christmas Eve, Christmas Day, New Years Eve, New Years Day).
- Other Requirements: Must include responses to specified questions in the application; local radius is 50 miles, considering local rate; candidates must disclose license and background check results upfront; no open license hits will be approved.
