Job Summary:
A licensed Physical Therapist (PT) specializing in Home Health services is sought for a position in Portland, Oregon. This role involves providing skilled physical therapy care to adult and geriatric patients within a home health setting. Candidates should be prepared to bring their license on the first day. The position requires at least one year of experience in Skilled Home Health and involves day shifts with a schedule of 8 hours per day, averaging at least 40 hours per week.
Job Details:
- Location: Portland, Oregon
- Shift: 8-hour days (Monday through Friday preferred) with minimum 40 hours per week
- Required Qualifications: Valid Oregon license; valid Basic Life Support (BLS) certification; at least 1 year of Skilled Home Health experience; experience with Electronic Plan of Care (EPIC), Rover (phone app), and Oasis; reliable transportation; COVID vaccination booster required
- Preferred Qualifications: Experience with wound care and outpatient clinics; proficient in OASIS documentation
- Schedule: 8-hour days, Monday through Friday; no weekend requirement indicated
- Additional Information: Travel within Portland and surrounding areas up to approximately 60 miles daily; assessments and treatments tailored to adult and geriatric populations; candidate must have their own transportation; occasional visits to multiple patient homes daily to maximize efficiency; adhere to strict clinical documentation and compliance standards.
- Certifications: BCLS (Basic Life Support) required; other certifications in wound care and manual therapy preferred but not mandatory.
- Experience: Minimum of one year in Skilled Home Health is required.
- Client System: EPIC Electronic Client system and Rover phone app used for charting and documentation.
